Dual Language Immersion (DLI) Interest Form

Dual Language Immersion (DLI) Interest Form for 2025 – 2026 Kindergarten Students Only. Please click on link or scan QR code to submit your interest in having your incoming kindergartner be in the DLI Lottery for Dolores Huerta International Academy or Redwood Elementary School. Available from February 3, 2025 to April 4, 2025.

Other Forms

Below are forms that some families may need to use to complete an enrollment with FUSD.

Physical Form (Bilingual)

• Students enrolling into Transitional Kindergarten (TK), Kindergarten and/or 1^st Grade need to have a completed physical form filled out by their child’s physician.
• Students enrolling from outside of the Unites States of America need to have a completed physical form filled out by an American doctor plus have the results of a TB test.

Declaration of Residency (Bilingual)

• Families who are able to provide an acceptable proof of residency within 30 days may fill out Section 1 of the Declaration of Residency.  This gives the parent/guardian an additional 30 days to turn in proof of residency to the school the student is enrolled into.
• Families who are unable to provide an acceptable proof of residency due to living with another individual/family may fill out a Declaration of Residency.  The parent/guardian fills out Section 1 and the homeowner fills out section 2 plus provides a copy of their picture id and utility bill.

• The Caregiver Affidavit allows qualified relatives of the student being enrolled to complete enrollment process. As long as the student is currently living with them in the same household, and they are not biological parent or legal guardian.
