Walk-in Process for New or Returning Students

Enrollment Process for Parents/Guardians who want to come in as a “Walk-in” (No Appointment)
STEP 1 – GATHER THE MANDATORY DOCUMENTS NEEDED FOR ENROLLMENT.
These documents must be presented at the time of your enrollment.
- Parent / Guardian’s Picture ID
- Proof of Residency
- Proof of Child’s Birth
- Child’s Immunization Record
- Child’s Physical Form or Proof of Upcoming Physical Appointment. This is needed only if one of the following applies to your child.
- Enrolling into grades Transitional Kindergarten (TK), Kindergarten or 1st Grade.
- Enrolling into FUSD from another country must have a completed physical and TB test.

- MANDATORY DOCUMENTS FOR TK – 12TH GRADE (BILINGUAL)
- MANDATORY DOCUMENTS FOR EARLY CHILDHOOD (BILINGUAL) – Students needing to be enrolled to be accessed for Special Education Services.
The following documents are not mandatory, however, if they apply to your child, please, bring in the most updated copy of the document. Some of these documents will assist us in placing your student in the correct program and/or school.
- Special Education Documents – Most recent IEP
- Foster or Legal Documents – Foster placement papers, Court Custody Documents, and/or Restraining Orders
- Approved Intra OR Inter District Transfer Letters
- Previous School Transcripts (High School Only) or School Student Profile (All Grades)
- Medical Insurance Card
Some enrollments may require additional enrollment forms to be completed for an enrollment. These forms are located under our Additional Forms Tab.
- An adult family member who is not biological or legal guardian will need a Caregiver Form.
- A family not able to show proof of residency will need to fill out a Declaration of Residency Form.
- A student needing a physical we have a physical form you can print out to take to your child’s doctor.
STEP 2 – VISIT US AT THE ENROLLMENT CENTER WITH MANDATORY DOCUMENTS.
All enrollments are processed at the Enrollment Center, 9548 Citrus Ave., Fontana, CA 92335. Parents/Guardians may walk-in anytime between 7:30 a.m. and 3:30 p.m. for grades TK- 12th or 7:30 a.m. and 3:00 p.m. for preschool.

STEP 3 – SIGN-IN WITH RECEPTIONIST.
Our receptionist will add you to our sign-in log and while waiting for our next available clerk to assist you with your enrollment, you will be asked to fill out 6 enrollment forms.
STEP 4 – COMPLETE YOUR ENROLLMENT.
We value your time and will have you to an enrollment window with one of our friendly staff members as soon as possible. Wait times vary and are determined by the number of walk-ins and staff members on duty.
